How did Walter Koenig die?
Walter Koenig
September 14, 1936 — October 25, 2026
North Hollywood, California
Walter Koenig died on October 25, 2026 in North Hollywood, California, aged 90. The official cause of death was cardiopulmonary arrest, due to chronic diastolic congestive heart failure; acute kidney failure contributing. Readers have raised one theory about how it happened.
Walter Marvin Koenig was born in Chicago to Lithuanian Jewish parents who had come through Ellis Island, grew up in Manhattan, and went to Grinnell to become a psychiatrist before deciding, at UCLA, that he would rather act. In 1967, in the second season of Star Trek, he was cast as Ensign Pavel Chekov, a young Russian navigator the network hoped would bring in teenagers; he wore a wig for his first episodes because his own hair was too short. He played Chekov in thirty-six episodes and in seven films, and he was ninety when he died, at home in North Hollywood.
Away from the bridge he was a writer as much as an actor. He wrote episodes of Land of the Lost and Family, three books including Warped Factors and Chekov's Enterprise, and a comic, Raver. On Babylon 5 he played Alfred Bester, a smiling, terrifying telepathic policeman, which many people who'd only known him as the cheerful kid on the Enterprise found the best thing he ever did. He spent decades at conventions, and was by every account kind to everyone in the line.
His son, Andrew, an actor, died in 2010, and Walter spent the years after speaking about suicide prevention. His wife of fifty-seven years, the actress Judy Levitt, died in 2022. He is survived by his daughter, Danielle.

The record
What is established, from the authorities and the people who were there.
Official cause: Cardiopulmonary arrest, due to chronic diastolic congestive heart failure; acute kidney failure contributing (death certificate, reported November 3, 2026)
He died at his home in North Hollywood on Sunday, October 25, 2026. He was 90. His representative said he "shared his kindness, wit, and appreciation with everyone fortunate enough to meet him."
He was cremated on October 30, and is to be buried at the same cemetery as his son, Andrew.
On November 3, TMZ obtained the death certificate. It lists the immediate cause as cardiopulmonary arrest, the underlying cause as chronic diastolic congestive heart failure, and acute renal (kidney) failure as a significant contributing condition.
His death came within days of that of Patrick Stewart, the captain of a later Enterprise.
What happened
The cause of death has been confirmed. The theories below are how readers understand it, and what people thought before it was known. Everyone here is anonymous.
The certificate says his heart stopped because of heart failure, with his kidneys failing too. What is diastolic heart failure, why do the heart and kidneys so often fail together, and what does that mean for how his last weeks went?

Heart failure that had been building for years, with his kidneys failing alongside it at the end
The certificate reads like the common, gradual end of a ninety-year-old with a stiff, failing heart: the underlying disease is chronic, the kidney failure is acute, which suggests a decline over his final days or weeks rather than anything sudden. He died at home, which fits a death people around him could see coming and chose to let happen there.

Losing his wife in 2022 mattered more than any line on the certificate
People who lose a spouse of more than fifty years have a measurably higher risk of dying in the following years, especially men and especially from heart disease. It isn't a cause a coroner can write down, and it doesn't contradict the certificate. Readers who followed him at conventions after Judy died say he talked about her constantly.

The confirmed cause carries this mark and stays at the top. Theories can still be added; they sort below it.
"Cardiopulmonary arrest" appears on almost every death certificate; it means the heart and breathing stopped, which is what death is. The useful line is the one under it.
Diastolic heart failure (now usually called heart failure with preserved ejection fraction) is the most common kind in people over eighty. The heart still squeezes normally, but it has grown stiff and can't relax enough to fill properly between beats, so blood backs up into the lungs and legs. It tends to progress slowly over years, with periods of breathlessness and swelling managed by medicine.
The heart and kidneys depend on each other. A heart that can't pump enough blood starves the kidneys; failing kidneys hold on to salt and water, which strains the heart further. Doctors call this cardiorenal syndrome, and "acute kidney failure" alongside heart failure on a certificate usually means that loop tightened in the final days. It generally points to a gradual decline at the end of a long life rather than a sudden event.
